Book Bag Request Form

Please ONLY use the form below to request book bags to distribute during your organizations' back-to-school efforts.

  • Due to COVID-19, this year we will be distributing the book bags via a drive-through distribution, date to be determined. There will only be one date for ALL distributions and someone from your organization must come on that day.

  • Submissions through this form are evaluated on a first-come first-served basis and are not a guarantee that book bags will be provided.

  • Please allow 7-10 days for a representative from The Children's Trust to contact you for further direction. 

  • If you are partnering with another organization please note that only one entity is allowed to apply per back-to-school event or series of events.
